i got a mail that my decision envelope for my application has been received at my vac in my country and is ready for collection. Should i be worried cause i haven’t even done my medicals. It is a student visa application though.

You'll have to wait and read the letter enclosed within the decision envelope once you receive it. The migration section at the High Commission of Canada in Ghana (or the country processing your application) should contact you by email shortly regarding your application.

What does your ECAS shows? Does it show Decision Made?

but the mail says “decision envelope on your application “ meaning it has been processed. i’m worried i got denied

Then it's either that your passport is being returned to you to reapply online because VACs aren't accepting paper applications or that unfortunately your application was denied.
