Answer to "Has this person ever used any other name" when name on passport and other documents is different

I have my name on my passport and my IELTS documents as follows:

First Name: X Singh
Last name: Y

On my WES reports, educational and employment related documents my name is as follows:

First Name: X
Last Name: Singh

Should I select Yes to this question. If so, what should I submit as the legal name change doc.
In case I submit no and provide a "One and the same name affidavit", will it suffice?

because I am also in similar situation and need to get affdavit for one and the same name but I don't remember that I have selected yes while being asked.

This is in the first form when we enter the Name. I am talking about post ITA forms.
But I want to confirm whether not having a surname on some documents counts as name change

I think in your case name is not change its incomplete mentioned in few documents.
I think in LOE you can mention about it or if need to be on a safer end get a affidavit done thats both are one and the same person and also explain in your LOE about it..
