Nope. Write India as country where u were born but Pakistani should be mentioned under nationality/ citizenship question.
Technically, your Indian nationality is renounced once you acquired Pakistani citizenship for migrants who migrated to Pakistan after partition. And vice versa
Yes. I was born in Pakistan and post partition migrated to India. In PR and citizenship and even in my Indian passport my place of birth shown is city in Pakistan and country of birth shown as United India and I have shown nationality as Indian.

Yes. One should refer the exact details. Place of birth will be India. In the status column & end date column, you can mention the details. Current Citizenship / Nationality will be Pakistan from the date of migration to Pakistan.

Place of birth can be different from nationality, so write the place of birth and nationality accordingly, If u held any passport or citizenship document/status from India then u have to mention it.
