Question 5 in Citizenship application form

Hello,

In the citizenship application form CIT0002E, # 5 question is: List all the names you have used, including name at birth, maiden name, previously married names,, nicknames.

If I have only one name that I have used all my life, do I still have to fill this question?

I have not changed my name. What should I write for Question 5 of Citizenship application.

My spouse has changed her name legally. What should she write for Question 5.

To be on the safe side, I think you should still answer that question and put your name in the table and list it as 'name at birth'. Even though I think it's for people who had a different name at birth and changed it later on. But according to the sticky thread post in this forum by @rajkamalmohanram which serves as a comprehensive guide, he suggests that everyone should write their name as name at birth. See below.

Q5: List all names that you have used, including name at birth, previously married name(s), alias(es) and nicknames.

• Whether you have a nickname / alias or not, everyone MUST answer this question with their name at birth.
• If you have used an alias / nickname or any other kind of name, then answer truthfully.

It CLEARLY says "including name at birth". Unless you never had a name in your life, which I think is quite improbable, you MUST fill this section in.

rajkamalmohanram has put trmendous effort in creating comprehensive guide but he needs to correct the information given for Question 5.

Here is the exact wording from the form's (?) i.e. help section of Question 5:
"We need to know all the names you have ever used in your life, so we can verify your identity. If you have ever used any other names other than the one being requested in your grant of citizenship application, print them in the chart. Examples: Name at time of birth, name before marriage, previous married names, married names, nick names or any other names you have used."

Note: I have emphasized on the point I am trying to make here by bold and underline words.

I would like to add one more thing here is they would want you to add all the names you have ever used means including your current name but that's the case being if you have used any other names before.

Yeah, that one line "If you have ever used any other names other than the one being requested in your grant of citizenship application" is what had me confused as well. Think of why you would want to explicitly state your name at birth if your name at the beginning of the application is the same and has never changed, it doesn't quite make sense. In my opinion, the reason the 'name at birth' option exists is for cases where you were given a name at birth by your parents and you later changed it to something else. In that situation, your current name would show up at the beginning of your application in the full name field, and you would list the name at birth in Question 5. I believe IRCC should remove the ambiguity by saying "name at birth (if changed/different from current name)" or something along those lines.
However, to be on the safe side, I have included my current name as name at birth in Question 5 as well. In the worst case, the IRCC officer will look at my current name and the name at birth in Question 5 and realize they are the exact same and understand why I put it there.

I will write spouse's name at birth in first row.

and

second row should I write her changed name. But in drop down menu there is no appropriate selection. She changed her name because, she did not had last name earlier.

I also believe that there should be a drop down selection for saying official name change.

Any suggestions.

Secondly. Is the copy of old IELTS test report (which was submitted at the time of PR) enough for English requirement.

The reason why I emphasized this so much is because of the nature of the question itself. "List all the names that you have used, INCLUDING THE NAME AT BIRTH, previously married name(s), alias(es) and nicknames".

I think it is safe to say that most people's current names are names given at birth. That is the reason why I mentioned it is important to mention the name mentioned at birth.

If my interpretation is incorrect (which "could" be the case) - then there is no harm, you enter a name given to you at birth and you are good to go. However, if my interpretation is correct (again, which "could" be true), then it'll save you a returned application.
