Self-Isolation Plan for First Landing - Any template to use?

Hi everyone,
I understand that due to COVID 19, a self-isolation plan needs to be presented to the immigration officer in order to complete the first landing process at the airport. Are there any guidelines or template which list the information and documents to be mentioned/included in the self-isolation plan.
I will be landing in Toronto.

Just a booking for your own accommodation consisting of your own room/apartment with your own bathroom, it should be for a minimum of 14 days.

Is AirBnB apartment reservation acceptable for self isolation or does it have to be a hotel/Motel?

It would be acceptable, but i'm not sure Air BnB is operating in Canada at the moment. You'd have to check.

Airbnb works fine, am isolating myself in one right now.
