You will receive an email. If you have CIC account and have linked your paper application then you can also see that in the account.
 
I was asked to collect my decision envelope at vac . Does this mean my application was rejected since I haven't done my medicals
 
Yes, not all country/ course/ applicant require a medical? where are you applying from and what course?

I think for Nigeria, medical is not necessary. I may be wrong but may be the ones whose application was approved may know better. If you linked your application online then you will be able to see the decision right away.

The honest truth is that you have be refused, since you don’t do upfront medical. Approval come after medical request for Nigerians.
