Should we sign IMM 5562E

Hey Y'all,

Received an ADR to fill up IMM 5562E.

The document mentions:

Travel History: A separate and newly completed Supplementary Travel Information form (IMM 5562) bearing an original signature.


IMM 5562 form doesn't have any section/box for signature. Should we just print, sign the bottom of the pages and scan it?

Hi Anderson,

I too received an ADR for this form? Did you sign the bottom of the documents or just uploaded it without signature.

Also, if there are any not applicable sections like no children above 18 yrs, did you leave it blank or filled it with "Not Applicable"
