How can I submit Bridge open work permit application?

Hi all,
Does anyone know how to submit the application of BOWP?
I saw some information from CIC web, and I think the apply way is paper only, but I can't find the address.
So I hope someone could let me know what's my next step of BOWP after I finish filling the form.

BOWP is on the list of people who may not be able to apply online

"may". that means if you run into trouble online.
BOWP can be applied online without issue
