I was asked to give sputum test done that was for three consecutive days . The last date was oct 16,2019. I got a call yesterday from the hospital to come back and give x ray for the second time. Should i get worried? Is there any chance of me getting visa?

The second x-ray should be done 3 months after the first. The results of the second x-ray will be compared with the first, to ensure there are no changes between then and now.

The panel doctor would have explained to you why the Sputum test was required, example, scarring on your lungs, history of tb, nodules on your lungs etc.

After my second x ray is done are they going to tell at that moment whether i ll be getting my visa or not or i ll have wait for few days more?

If your application is still being processed and you pass the medicals, you'll get a Medical Pass update in your account 1-2 weeks after the panel clinic submits your x-ray and Sputum results to IRCC.
