What are your ground for extension? What is your exact scenario?

I am currently working in Canada from last 18 months , I am applying for the extension of my work permit which is valid for another 4-5 months . While applying it online, I got through the medical exam option , so wondering if I have to undergo the medical exam or not. I have been in canada from July 2018 and haven't lived anywhere else during this tenure.
