Can referrals letters be in form of affidavit from supervisor for SINP

Hi everyone, just wanted to know whether the reference letters with the job profile details can be given on an affidavit. As my company letter head document does not have the details like working hours, each and every job explanation etc.

An immediate revert will be appreciated.
