Hello! I hope someone can offer me some insight. I have read through so many threads, but I still fell as if there is some gaps in my understanding, and I have questions!

First, a little bit about us...my husband(American) and I(Canadian) met in Southeast Asia 4 years ago where we worked and lived for the same NGO. We got married July 2018 in Canada. Now we plan on moving back West at the end of the year, and have decided for my husband to apply for PR in Canada.
Our plan was to visit Canada for the holidays, send in the application at that time under the category of "Not living in Canada, Family class", then he would go to his hometown to wait. I would head over to visit him while we wait.
We talked briefly with an Immigration adviser and we were told that we cannot apply from out of the country, and that both of us would need to be in Canada during the application process (my husband would apply for a work permit). This really confused me!-- can some one shed some light on this? I am sure there are others who have applied from out of the country! I technically would only be visiting him, not living in American while we waited... could this possibly be a problem?
Another question I have is if it would cause a problem to change my last name during my husband's application process? We are trying to decide if it would be best to start the application right away from here(Asia), or if we should wait until we are in North America. If we were to wait, I plan on changing my last name.
Lastly, when we visit Canada for the holidays would it be necessary to let Immigration know that we plan on applying for PR while we are visiting? Would a plane ticket out be enough to convince them that my husband does not plan on staying forever without going through the proper process first?

I will shed some light: the "immigration advisor" you spoke to is a f***ing moron. Don't go back there.

It is perfectly fine to apply outland. There is no reason to wait until you come to Canada. Apply now. You guys can visit Canada while the app processes. You can go visit him in the US while the app processes.

You can change your name whenever. No effect on the app.

If CBSA asks, yes. If they don't ask, no. If you already have the PR app submitted by then, they will see it in the system.

You certainly can apply from outside for your husband to be an outland applicant. this lets him stay and work outside of Canada till process is done and he decides to immigrate. Yes, you will have to prove the intention of moving back to Canada in this one.
if you decide to go Inland, you both will have to be in Canada and the applicant will not be able to travel out during the course of process.
With regards to name change, it is not an issue. You will be required to keep IRCC informed of any changes to your circumstances so if the names changes after you've filed the application, all you got to do is update IRCC of the same. (this is purely as per my knowledge and would suggest to further check on this).
When you guys visit you don't need to voluntarily spell out anything, but if asked do not lie as well. You anyways would have all the supporting docs like air ticket and work details on this.
