Never heard that, who told you that ?
I read in some threads only. person should be in Canada during surveillance period otherwise surveillance is not considered completed probably doctor might call for follow up visits.
Do you know someone personally who have done surveillance with soft landing and still received pr card recently

There is no restriction on travel during medical surveillance ( both domestic and international). Requirement is to inform your current address.
As you because PR at the time of first landing, PR card is issued to you.
Usually a follow-up visit is asked after 6 months of test, you can go to clinic if you are in Canada or request for date extension.
Have you received your COPR ?

Ya received copr last week, planning to visit in November first week but due to this medical surveillance issue, have not booked tickets as I cannot stay back in Canada for long and have to return. I m worried as its clearly mentioned on ircc website that in case surveillance is not over and person leave the country, immigration officials can stop while returning to Canada for further questioning

Please share your experience once you finish ur surveillance. Will be travelling later on.

You will likely be asked to call public health when you return to Canada and settle for good. Public health follows people with communicable diseases who live in Canada in order to monitor any potential spread to the general population.

No they are completely different things. You must still call public health and follow through with your meducal surveillance. The medical surveillance will likely just continue when you settle permanently. It isn't as big a deal as you are imagining. Public health will likely retest you again.
