Received IMM 1017 - but it has a mistake! How to handle this?

I just received IMM 1017 medical request, but the "Country of birth" field is incorrect. It shows my country of citizenship as my country of birth, but they're different.

How do I ask CIC to fix it or should I not bother with this and simply book the appointment at the physician and let the physician know instead?

Advice would be appreciated.

Do the medical. Send a Webform to IRCC about the error.
