medical Exemption

To be honest im not 100% sure what you wrote there, but can you not get tested again and do another set of medicals and these medicals show you are now clear?

i exempted because of Problem called "X" but i never had this X problem
SO how i exempted ? easy ..it is para clinic ,i say in other words,this kind of problem depends on situation that time Yes 3 days i did not sleep so it shows wrong report ,
Do i have experience show as document for support ? Yes , i repeated same task as before ,easy
From my perspective if i have now problem who can prove ?
Yes i had headache ,yesterday i had Stomach ache,1 year ago i was blind for 1 week
Yes i had 1000 disease but how they want to prove and How they can
of course ask for test NOW i am OK without any problem but
4 years ago i wasn't healthy
does anyone reject you based on what you were in past?or what are you in current?
who want to judge You based in Your past ?

i am eager if some one can answer this

when i was child i was very badly[my mom said] so what is different between this problem i exempted because of X but now there isn't problem
does Canadian officer ask you everything?
for example will they ask why you got 12 in mathematics ?why you got your bachelor in 10 semester instead of 8 ?

I would say its more important what you have now and how healthy you are now vs the past.

Another thing to consider though is that if you have lied to trick your govt in the past, what's to say the documents you provide for the rest of your file are accurate or are you misrepresenting yourself to trick Canada.

Any case nothing anyone here can do, just follow the steps of your application and hope for the best.
