Only work hour is mentioned, for salary I submitted the offer letter and payslips

Manager's name is not mentioned. Some HR has signed whom I don't even know. But somehow it didn't matteri I got the passport request

You can apply for it through exit helpdesk. Mail to them, they will send you a format which you have to fill and send back, they will send it to your manager, once he approves, they will issue it

Once manager approved u can get it immediately...
