Q. 10 and 11

I am signing my application for July 19. So if I was working for a an organization since let say Jan 2014, because they have only asked for 5 years history should I still write the original month or start July onwards? I am confused, I just dont want to have the application returned for false/ incomplete application and the same for the address.
Any ideas/suggestions would be appreciated.

I listed my actual start dates for work and addresses even though they went beyond the 5 year period.
