Medical: Complete or provide proof of completion of the Immigration medical examination
Ø
Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature explain why you lived at two different address, one in Germany and the other in Philippines
Ø
Police Certificate: From each country/state where the residence period has been six months or longer since the age of 18, provide an original police clearance certificate from Germany

Pls guys just to confirm and I am a bit worried.
This was the last email I got from the visa office regarding documents I need to provide. And this email was sent to me the same day I was requested to do medicals. I already recieved pre arrival since May15th. So my question is. Does this mean I have to send them my passport togrther with this documents, or I jusy have to send the documents listed there alone.? Because I already uploaded this documents stated there, I already submitted it on CASE ENQUIRES ACCRA. So pls jusy to confirm. Does this message means I have to send them this papers including my passport or just this alone. Becsuse when I checked on ECAS, it shows that medical has been recieved. So I am a bit confuse and I don't wanna make any mistake or delay. I will be glad if someone can enlighten me.
