Question about the UCI #

I created a EE profile back in 2018 and it was declared as "Profile ineligible". Next I created a new EE profile with updated language test scores and got ITA. I see a UCI number in my profile summary.

The UCI format is CAD0000XXXXXXXX
but
the UCI field in the PR application asks for 0000-0000 or 00-0000-0000 as shown here

Questions:

1. For the medical checkup my doctor asked me to give the UCI #. So I have to give this # right?
2. Secondly, I received my ITA and in the PR application, it does ask me for the UCI #. I have to provide this # right?
3. I am confused what to fill in the UCI field because of the different formats as mentioned above.

Please advise.

The UCI number needs to be stated in case you have applied to IRCC prior to your current application (Ex. Visitor visa, Study permit, Work Permit or a previous Express Entry application that was accepted by IRCC). once IRCC processes an application, a permanent UCI number is assigned to the applicant.
As long as you remain in the EE pool, you are a 'candidate' and you become an 'applicant' once you submit your application to IRCC (after you submit your e-APR). In your case, since your profile was deemed ineligible while were still a 'candidate', you were never assigned a UCI number and this does not apply to you.
Back to your questions;
1. You can carry a copy of your ITA to your doctor. All they need to know is that you are doing an upfront exam for Express Entry as a FSW . Also, make sure that they select 'Non-EDE' (Non excessive demand exempt) type of exam.
2. You can leave this field blank.
3. The UCI format that appears in your profile is a temporary number and shall be revised when your application is taken up for processing.
