Hi All,

It's been 7 weeks since AOR and while I was expecting update to the medicals and biometric I got an ADR.

We have been asked to submit IMM 5669 Schedule A within the next 7 days.

My question is when is the declaration sought from applicant? What's it that they want to know/confirm? Where can I find the schedule? I found a document titled imm5669 E, is it the same? Any tips that will help me get through without further delays? Lastly will the Medical/biometric things get held up because of adr? Please help with your valuable understanding of the process. TIA!

I got adr for the same form 3 days before my medicals passed and I got biometrics letter. I ordered gcms notes and I saw that I passed eligibility less than 2 weeks after I submitted it.

Perhaps there are gaps or overlaps in your address or personal history that they want to confirm. Not really sure.

Hey my case was same as yours. After 44 days of AOR I got ADR request 5669 . I am principal applicant. We both me and my husband downloaded form filled it and signed it electronically nd submitted. On next day again we got Adr request and they required my husband's original signatures. So again we both filled it with original signatures and next day got our medical and bio letter,. Nobody knows why cic asks for it. It is basic declaration nothing to be worried . In my case I guess they asked for it because in statutory questions I answered yes as my husband's visitor canada visa was refused once . U shud not worry and read Adr letter carefu , if original signatures are asked sign it scan and upload. U can upload it in your uploaded documents section. There u will find option from ircc . Good luck

Very relieved after reading your message! Thanks so much.
Would you be able to give me the link to that form? I found an IMM 5669 E...is that the same? Also should I take a printout of the pdf file , fill up the details along with signature and upload? Is that the process?

I believe that they like to keep it as a record when they see a long and complicated background history/multiple addresses. My guess is that they simply want a physically signed document from the applicant as a proof that the applicant has personally verified all the addresses and dates and it isn't simply a case of some agency incorrectly filling wrong addresses.

In almost all cases this is just a precaution, the only case you should be worried is if you've intentionally falsified any of the information which I'm sure you haven't. So don't worry. This also doesn't affect processing time as it is very common to receive IMM 5669, we did not receive it on our application but my brother in law and his family did when they applied last year.

Your comments are very reassuring. I think you're right since my spouse and I have a very complicated address history..we've lived in multiple addresses in two different countries and there's a lot of overlap as well. Hope our adr response is accepted.

In the form we are asked the number of years of education for Elementary secondary and university. Does primary and secondary education in Canadian context mean grades 1-6 and 7-12?

I also got the same ADR. I completed and submitted the doc. I received biometrics request letter the same day I submitted the ADR. Please note when submitting the ADR, make sure after uploading, you click next till you get to the place where you sign the declaration and you get a message; "document submitted successfully". Don't just upload only and think you have submitted.

AOR- 8th Feb 2019. Completed my biometrics yesterday. So waiting patiently now. I am giving myself till August in order to save myself of unnecessary pressure from expecting an outcome soon.
