I can only speak for someone who has sponsored their spouse in the family class (not sure what family member you are sponsoring or applying with), but in our case, we submitted our application and my husband was sent a request by CIC for his medical exam that he had to take with him. The dates for our timeline are included in my signature so you can see when application sent, received, request for medical etc.

They will request the medical after you apply.
