I need to request my certificate of departure

I have misplaced my certificate of departure that i got after being deported from Canada, i guess i need it to be able to cross the border again, its been over 12 months. Is there an application i fill out? This is all very confusing to me, any help will be very appreciated, thanks!

following up on this. Did you need the certificate of departure?
