Passport country of issue confusion

I got my passport renewed recently from New York consulate. But I am from India, so when you fill up the Express Entry profile, what should I give my "Country/territory of Issue" as??

I gave it as "USA" in my first express entry profile and after few months was turned down as "ineligible profile".

I started a new profile, therefore I am trying to check on all possible questions.

Did IRCC state that the mistake in the country of issue was the reason for your Ineligible profile rejection or did they not give you a reason?
