I just received my GCMS notes, and looks like I have a "Review Required" scenario mentioned near the last couple of pages. Can someone please shed some light on this?

There are 2 stages of check before the final officer review.
The program assistant do preliminary checks, R10, CRS check, etc then reccomend for review any documents not clear to him. This recommendation is for the case analyst to look into. You don't have anything to worry if the case analyst didn't set any document for review.
Under your assessment, if eligibility is showing review required, a big reason to worry but if recommend pass then you're OK.

Does GCMS notes also create Ghost Update? I ordered GCMS notes on 18-Dec-18 and today I see a Ghost Update, not sure if it is due to that or something else. The current status field says 'Application/profile updated' but when I went inside nothing changed.

It's normal these days. No need to freak out until the 6 month mark is passed. I know the wait is excruciating, but try not to panic. We all are in the same boat.
It's been rather a drought since the new year. Hoping for a PPR shower tomorrow after the scheduled technical maintenance.
Your application could be in the process of being finalized.
