I have a family member that needs me to come and take care of them for a year while they recover from a medical condition. I am considering doing this, but need some information first. I currently live in the US with a green card and am a Canadian citizen. I am married to a US citizen, who will remain in the US while I care for my family member. I would like to be able to continue working my job remotely while I am in Canada, but not sure how that would work. My medical would not transfer over so I would need to obtain medical in Canada. Can I work remotely in Canada and still maintain my green card holder status?

You should post your question regarding your green card status to a US immigration forum. You will likely need to apply for something like a returning resident permit in order to be outside of the US for that long and still retain your green card.

In terms of having medical coverage in Canada, generally speaking you have to return to Canada and live here for three months again before you'll qualify for provincial health care coverage. So you should purchase travel insurance to cover any emergencies for the first three months (you'll have to pay for any non-emergency care out of pocket).

Working remotely is no problem. You'll need to file taxes in Canada and declare this income (as well as file US taxes).
 
You'll have to speak about payment since they should be paying you based on your Canadian address. Many are asked to set up their own company and act as a consultant for tax and processing reasons for both you and the company. You'll also won't have extended health benefits for things like dental and medication.
