Difference in given name in passport and name in birth certificate

Hello folks,
Queries :

1)Is birth certificate, mandatory document for pre/post express entry process??

2)Can someone please help me with this case:
Given name in passport = Tyler Durden, where Tyler = Name & Durden = middleName/fatherName. Name on birth certificate = Tyler. Do I need do a affidavit (country: India) stating that Tyler Durden and Tyler are the same person.

Birth Certificate is only required in case of PNP applicants and kids.
