Ghost update is when ur application status shows updated but you dont see any updates at that moment... usually such updates follow DM's in a day or two

I submitted my passport in Bangalore VFS on 30th October & today the status has changed from “being processed in Newdelhi VFS” to “dispatched from New Delhi VFS on 16th November”.

I guess not. It will only show "medical results have been received" on ecas even though medical is passed.
Also, ecas doesn't display any information regarding file transfer but one gets an email confirming the transfer of the file to local visa office.
