This could happen when the name, father's name and date of birth matches and US info says the visa was refused. You can prove by sending the expired passports, and explaining them that this has never happened, and there could be some misinformation. You can state all the passports that you have had, their passport number, expiry dates and their bio scanned pages for their reference, stating that this is not true and must be cross checked as you declare that you have never ever applied for US visa and the information CIC has received is incorrect !Hi,
