"I have been accepted for 'Master of Applied Computer Science' at Concordia University for Fall 2018 Intake. I applied for a Study permit on 25th July (Online, General Category).
On 18th August, I received a negative decision for my application. The reason stated in correspondence letter is "You have not submitted satisfactory evidence from the prospective educational institution confirming that your admission is unconditional". Here are the conditions stated in my application:

1) Submit the official IELTS Score - Acted upon it and had submitted the IELTS SCORE. Attached the IELTS scorecard and Proof of University receiving the document along with the application.

2) Submit Transcript and Provisional Degree Certificate as a proof of successful completion of Bachelor's Degree - Had attached the mark sheets for all 8 semesters and also attached the provisional degree certificate. I wasn't able to send the transcript and provisional degree certificate by the time I had to apply for the visa as I got the Provisional certificate pretty late. Hence, could not attach the acknowledgment of the university for receiving it.

3) One more condition stated that admission is conditional upon successful completion of a prerequisite course within 12 months of the start of the course - Didn't address this as it was very well the case that I could have waved off this course and wouldn't have to take the course at all.

My question is what could be the possible reason for the remark, not attaching the university's acknowledgment of receiving the transcript and provisional degree certificate OR the prerequisite course.

My friend with the same exact conditions as mine, in terms of sending transcripts and in terms of the same required prerequisite course, got the visa approved on August 15th, 2018. Apart from that, the Quebec government also issued the CAQ for me, which I think would be issued only after reviewing my letter of acceptance. Moreover, my offer letter clearly states "this conditional offer letter is enough to apply for the CAQ and Study Permit". So, what could be the reason for refusal? What different should I do for reapplying?

Any comment/suggestion that could help me solve this problem is most welcomed and thanked for. Should I try to review the decision?
 
Given the situation, there is no way you can re-apply for study permit for Fall 2018. So, first thing you should do is to get your program deferred to Jan' 2019 intake.

In the mean time, try to remove all the conditions and get a new "Unconditional Letter of Acceptance" from the institution. The visa office doesn't wish to see any kind of conditions in your LOA.

Will remove the first two conditions. But the third condition of passing the prerequisite course is something that I could complete only after reaching there. Will it be an issue while applying the second time?

Your situation is similar to the applicants who need to fulfill the ESL requirement before starting the actual program. In your case, the Uni has put condition of prerequisite course before the actual Master program.

In such cases, it is clearly mentioned that CIC will issue study permit for the duration of prerequisite course. Once you have obtained the pass marks in prerequisite course, the Uni will issue unconditional letter. Later, you need to get your study permit extended using the unconditional letter.

Note that CIC mentions that student don't need study permit for program less than 6 months. I believe your prerequisite course will be less than 6 months. Hence, you may not need study permit initially. But this can be expensive affair with lot of hassle, as you need to first travel for prerequisite course, then come back to home county and thereafter apply for study permit.

Safer bet is that you apply for study permit for the duration of prerequisite courses only. Later, get your study permit extend for the duration of Masters program. In your SOP clearly explain the scenario. If you find this tedious then approach some experienced and authentic consultant. Even you can visit nearest VFS office for consultation and they may help you out.
