If a permanent resident is already 7 months pregnant when she lands as a PR, will she be required to pay the full price of hospital costs when she gives birth, or will she be eligible for provincial insurance (in BC)?

Likely somewhere between $5K and $10K. However if there are any complications, it could be a great deal more than $10K. If you want to reduce your expenses, you can try seeing if you can find a midwife and have the birth at home.

In BC you must wait the remainder of the month you land as PR, + 2 additional months before being covered by MSP. So depending on what date you arrive in BC, the total wait time could be as low as 2 months plus a day (if you arrive on last day of a month), or as long as 3 months (if you arrive on 1st of a month).

As mentioned, you should budget $5,000 - $10,000 for any exams/ultrasounds during pregnancy and a normal hospital birth. Amount may skyrocket for things like a c-section, or for any other complications requiring an extended hospital stay.

Thanks Rob_TO!
What if she has landed a few months ago (due to work in another country), but left the country after a few days after landing? Can she buy a different insurance until the provincial insurance kicks in?

No. No private insurance will cover pregnancy or delivery related costs when the person is already pregnant.

If she left the country so no longer meets the residency requirement for healthcare, she will need to start waiting period from the beginning after returning to reside permanently.

If you want to qualify for maternity payments you also have to work 600 hours so you may want to consider that.
