Read within your link for this para:

Get your exam before you submit your application
Are you applying to sponsor your spouse, partner or child?
A new application process for the spousal sponsorship program came into effect on December 15, 2016. Spouses, partners or dependent children can no longer do an upfront medical exam. Wait for our instructions before going for your medical exam.

This is called an upfront medical exam. To get an upfront medical exam, contact a panel physician directly.

Once your medical exam is done, the doctor will give you a document confirming that you got a medical exam. Include a copy of that document with your application.

If the doctor works with the eMedical they will give you an upfront medical notification printout.

If the doctor works with a paper system they will give you a copy of the IMM 1017B Upfront Medical Report form.

Attach that form to your application before you submit it to the visa office. If you apply online, you must upload that form before you can submit your application.

Only when you sponsor your dependants later on after becoming a PR ( spousal application), upfront medical exam is not allowed.
Otherwise primary and rest all applicants can go for upfront medical exam. I did it the same way for your information.

All of the above information is in reference to permanent residents applying to sponsor their families not for express entry applicants. Express entry applicants are required to wait for instructions.

I wanted to know about the appointment system for the medical exam and asked my panel physician and part of what he had told me was I needed a UCI or IME to make a reservation. That panel physician uses E-Medical, but each to his own.

I think you are misreading it. Only the yellow highlighted section is for sponsorship.

Well the OP is right and we can get upfront medical test done without ITA. And also after you get ITA you will need to get the medicals done and not wait for instructions from IRCC (this was followed before at the time of paper based applications).
Emedicals are attached with your passport number so there is never a need of a ITA letter, however panel Physician in India always insists on this as they don't want people coming in without ITA and take their time whom they can give to people with ITA.
Otherwise its your choice, I will suggest you to not wait for instructions as they may never come.

This is the response that I got from the place where I intend to take the medical exam.

Dear Visa Client,

In order to schedule your appointment you MUST provide:

VISITOR (up to 6 months) Visa Applications) http://www.cic.gc.ca

Please prepare for your Visa Medical Examination:

1) No alcohol or energy drinks at for a least a week prior to your appointment,

2) Be well hydrated (3 liters of water per day for 3 days prior to your appointment),

3) Please have some light breakfast on the day of your Medical Exam,

4) If you take prescription medication in the morning BE SURE to take them prior to coming in.

Must bring with you:

1) A valid Passport (a National Identification Card (government issued picture ID) may be used ONLY if your Passport is not available

2) A print out of the Referral Letter (unless you are having an Up Front Medical Exam)

3) List of any prescription medication you are currently taking

4) If you require a reading prosthetic (glasses, contact lenses, etc.) MAKE SURE to bring them with you.

The whole Medical Exam is broken down to three parts: Using E-Medical System

1) Pre-Exam – (20-25 minutes) where the Client’s Identity is confirmed, the Declaration is signed, and your Facial Photo is taken. At this stage, we check your initial Blood Pressure and collect the fee.

2) Required Blood Work and Chest X-Ray.

4) Once your examination is submitted you will receive a E-Medical information sheet showing that you have completed your examination and it was submitted to Canadian officials.

Please contact our staff if you have any questions or concerns about the Medical Exam procedure. Please note that we may answer some Immigration questions but we are not authorized to explain or assist in your specific situation. For detailed information on your application, please contact the assigned immigration officer at IRCC (email address).

Not according to this:

https://www.canada.ca/en/immigration-refugees-citizenship/services/application/medical-police/medical-exams/requirements-permanent-residents.html

It is explicitly stated to wait for instructions after submitting an E-APR.

Even when you contact a designated panel physician they would tell you to provide IME or UCI.

Upfront medical exams are only for students/workers/visitors.

And if a spouse/partner or dependents are being sponsored, you also have to wait for instructions starting from December 15 2016.
You should not be waiting for instructions as this delays the process. Ideally should go for Medicals post ITA.

I guess CIC will decide whether to accept that or not. You can take your chances, you can always explain this in your LOE that you wanted to save time etc.
There is nothing like that. Do not demotivate. Whatever he has done is absolutely normal.

I'm going to actually send an E-mail to the panel physician and ask him to corroborate what he said that a UCI or IME is required and that only students/visitors/workers can get an upfront medical exam.

I will, however, need some more info.

Those who have had their medical exam done post ITA, did you get instructions/letter or an IME or UCI or simply mentioned you had received an ITA and are required to take the medical exam ?

From the CIC website it only states one can take a medical exam prior to submitting e-APR but not prior to receiving an ITA. I want my challenging of the panel physician's requirement that I have a UCI or IME to be rock-solid.

The place I went they didn’t even ask me whether I had received an ita or not and they are designated by Cic

Again for one last time. You can do medicals before ITA and it is perfectly legal.

UCI/IME requirement for medicals is a pre-express entry rule. For express entry, medicals can be done without UCI/IME.
