Passport Application Questions - Child

Hi,

I was filling out my son's passport Application Form. I have a question:

Is it mandatory to fill the "Other parent or legal guardian" part in the Application form? My wife is hospitalized since August 2017 and it is hard to bring her signature from the Hospital. Is it okay to ignore that section? If not, then I have to wait to get an appointment from the Hospital for her signature and in the address field, do I have to provide the address of the Hospital or our Apartment address?

I have similar situation. I'm single mother.
But in my case, the father is living in my home country and it will take a lot of time for him to return the form by mail.

What should I do if the other parent is deceased?
If the other parent is deceased you should include the following with the application:

• death certificate
• child's long-form birth certificate with the names of both parents

In my case I think it is better to have my wife's signature from her Hospital and put her Hospital's address in the Address field.
