Indian PCC Query from SFO/US Consulate

I am in process of getting PCC from SFO consulate, and I have few queries regarding "personal particular form'. It would be great if any of you can help me with the same:

(---) 8(b) Present Residential Address & Tel. No, along with Police Station and residing since
This will be my current US address with US police station name ?


(---) 9. If you have not been resident at the address given at COLUMN 8(b) continuously for the last one year,please furnish other address(es) with duration(s) resided (Please furnish an additional set of P P Forms for each address with Police station.
I am staying in my current address from last 2 months only. So i need to furnish additional addresses for last 10 months only ( to complete 1 year detail ) or since i have arrived in US(3+ years) ?

Here "applicant’s locality" is referring to permanent residence in India or present residence in US?

Only 1 year detail is required. Hence need to furnish 2 PP forms [( Current-2 months + old ( 10 months) ]
