Help in filling the police clearance form(PCC) from USA for Canada PR

what should be filled in the blank below: India or the respective immigrating country as in my case case I will use this for Canada?
o Police Clearance Certificate for _____________________ (Country)  For PPT issued in San Francisco $25  For PPT issued in countries other Than San Francisco $25

Is the below field mandatory? Can I leave it blank since I am not working currently? And in my husband's case I believe it can be his office address, please confirm.

In the personal particulars form, Should the details of my permanent address be that of India and is it ok if the address is not same as passport address?

1. Mention country as CANADA (since you are applying the PCC for Canadian purposes)

2. Husband's office address would suffice. You can mention in your case NA (housewife/home-maker)

3. Permanent Address: should be an address in India, I gave my parents' home address. And my address on passport is different so it didn't matter.

Pro-Tip: The fee charged will be $25+$3 = $28 ($3 is the fee for some development fund)
