co-op work permit medical examination

hello I am a nursing student who is going to apply for co-op work permit. I might need a medical examination and I heard that it can be done anywhere in the world with panel physicians. But co-op work permit has to be applied in Canada. I wonder if I can get the medical exam in a different country and apply it there even though co-op is an inside Canada application. I am going back to my country over the summer and want to apply it there via online.

or can I do medical exam before I leave my country and apply co-op upon arrival to Canada?

It does not matter for the medical if the app is "in Canada" or not.

one of the eligibility questions ask "will you be getting a medical exam before submitting your application?" and if I say no, but still say yes for "request for medical exam", would CIC will send me instructions how to do medical exam?

okay I have some reasons...
first because I am not sure if I can apply co-op outside Canada as it is an inside Canada application
second I have a friend who is also a nursing student applied her study permit extension along with co-op work permit, but she was not requested to do a medical exam. the only difference between her and me is that my friend is applying co-op and study permit extension together and I am only applying co-op. so I would like to know whether I need a medical exam or not.

Were you able to find a solution? I'm also a nursing student. Do I need coop work permit even for my 1, 2, 3rd year clinical too?
