Help! Have you ever been refused a visa or permit, denied entry or ordered to leave Canada...?

My bother who has mild mental retardation was a dependent in a family sponsorship application for my mother (principle applicant) back in 2011, but my mother could not attend the medical exam due to her health condition and as a result her immigration application was refused.

I want to invite my brother for a visit to Canada. What should we select for "Yes" / "No" answer and in details for following two questions on my bother's visitor visa application.

1) Have you ever been refused a visa or permit, denied entry or ordered to leave Canada or any other country?
2) Have you previously applied to enter or remain in Canada?

Does it apply if a Visitor visa refused due to insufficient documentation?

Doesn’t matter what the refusal reasons were, if you have applied and had a refusal, the answer is yes.
