Hi everyone,

I received my ITA yesterday and now working on my PR application (post ITA).
Under personal details;

In names;

On my passport I have First Middle and Last name.

Masters degree, middle name is missing.

My national ID has another name in addition to First Middle and Last names. I have not used my national ID for the application. I only provided my passport.

Given the above information, do I need to answer yes to the following question; If so what are the names I should mention under other names.

Has this person ever used any other name?

Answer is No. My degree certificate does not have my middle name too. I only added a simple LOE for that, not even a notarized affidavit.

Is there a format for Letter of explanation? Do I need to sign at the end of the letter?

On a different topic, I just completed my application for residency. I am trying to upload my documents but I am confused which document is which.
For an example;

Employment Records (required)
Employment Records 2 (required)
Employment Records 3 (required)
Employment Records 4 (required)

Police certificate (required)
Police certificate 2 (required)

Those are the different employments you mentioned in your previous page. Need to show proof for all employments that you claimed points for.

Police certificates are for the different countries that you mentioned you might have stayed for a duration of more than 6 months.

You must provide proof of work experience for your current job and for each past position you listed

So is it a must to show proofs for my current job even if I havent claimed points for it?

I have a similar query...all my documents including marksheets, bankstatements extra mention my name as Hari Krishnan while only the passport has the full name mentioned as Hari Ramakrishnan. Do I need to produce an affidavit for multiple name or just a self declaration / LOE will suffice?

letter can be addressed as Dear Case Officer, .....

On ur other query, I am really not sure but do you mean you are planning to show that you are unemployed as of now?

I'd say, go for affidavit. Doesn't cost much and gives peace of mind while application is in progress ;)

I indicated my current job on my personal history but not on the employment history as I did not claim points for that.

I'm in a similar situation as well.

My last name on Passport and other govt issued IDs is: Gadhavi
My last name on educational credentials, WES assessment, and work-related documents is: Gadhvi

Noticing, that my last names differ by an alphabet a, should I answer 'Yes' to the question 'Has this person ever used any other name?', in my PR (e-APR) form?
If I answer yes, what should I select for 'Type of name'? Should I select 'Birth' or 'other'?

Any suggestions?

Hi, I am applying for my Wife's PR.

Now her Middle Name has a discrepancy. Her Father's Name starts from W. While in all her documents it starts with V. Both has same pronunciation in our Native language (Hindi). Do I need to give an affidavit for this? Is there any standard format for Canada Visa application?
