Have ppr : no appendix needed but they ask for a piece of paper with info

So i have ppr from paris. No appendix. They just ask for a sheet with eyes and measurements. Very vague.
Are:
Name, dob, uci,height, weight, eyes good enough?
And just the pa needs info on it? Not spoise and stepdaughter who are already citizens.

maybe for Paris VO this is different. I submitted inland and they asked me only address. No Bio info
