Hello,
I am currently working in U.S.A for the last 3 years. Within the last 3 years, I have had different roles and titles. So, when filling the express entry profile, should I put the current active title under work experience or should I list all the roles and the dates? Also, what are the documents that I need to ask from my employer? Does just a general work experience letter for the last 3 years work?

You would need a reference letter which follows IRCC's guidelines: google express entry intake.

Regarding your question about how to fill your profile, whether you need a single entry or several depends on whether you had a single NOC or more than one during your employment.

My experience letter only states my last role and location whereas I have worked in 2 different locations within the same organisation. It was the same NOC throughout my stint except that I took an internal transfer. So my address history and work history (as certified by the letter) will obviously not match.

I can just explain that in my LoE but will that lead be construed as misrepresentation as they have no way to verify if the NOC of my previous role was same.

I'd appreciate your thoughts on this situation.

Get a new reference letter that states the exact periods and locations were you worked.

My reference letter does not have salary and benefits due to company policy. I am planning to include my annual revision letters to provide this information. However, there is a fine print on these letters that say "for internal use only, not valid unless duly authorized". I am trying to get the letter authorized by HR, but not sure if they will do it. Will it be a problem if I submit this letter without authorization? Please let me know, thank you!

If they refuse to do it then you can also provide pay stubs to prove you were getting paid throughout your employment. Remember that what IRCC is interested in is that you're getting paid not what your salary is.
