Open Work Permit - Spousal Sponsorship

My wife is sponsoring through the inland spousal sponsorship.
I am currently on a visitor permit

This means that I can apply for an Open Work Permit.

This requires form IMM5710e

Some of the questions are confusing me and I hope you can
offer some guidance. I cant get through to the CIC call center

I had a 3 year open work permit after graduating university.
This was back in 2007

So one of the questions is this

Check the appropriate box or boxes to indicate if you are applying for:

• A work permit with the same employer
• An initial work permit or a work permit with a new employer
• Restoration of your status as a worker
• An initial or an extension of a temporary resident permit

Which one should I choose?

Hey @kayvee, what did you end up doing? My fiance is about to go through the same process.
I'd really appreciate any insight you could share.
