Would like to share some experience.
1. Reading - Most of the test takers are over confident that they have marked their answers correct, but, in reality questions are usually twisted and the answers are not as straight forward as we think. Thorough practice can help us cross the obstacle.
2. Listening - Answers should be exactly what we hear in tape. Dont try to add or subtract any word. Also, try to look out for singular or plural word by fitting your answer and reading entire line.
3. Writing - Practice, practice and practice. Correct logical flow, use of different grammar structures - both simple and complex, Moderate level vocabulary can help you achieve a band 7 minimum.
4. Speaking - Just relax and face the examiner as you are talking to a colleague. Keep tone appopriate, use good vocabulary, connecting devices can help you excel in this.
I hope this can help all of us to get desired bands. Do share your experience as well.
