The RA reference letter requirements are the same as any other reference letters needed by CIC, as follows:
1. Should be on university letterhead
2. Include employment dates
3. Hours worked/week
4. Stipend information
5. Job responsibilities.
6. Signature of supervisor
7. Supervisor contact information
Below is a sample format, which you may tweak based on your requirements:
XXX was employed at YYY University by the department of ZZZ from Date to Date. XXX was hired part/full time (XX/hrs per week) as a "Title" for a stipend of XXX/month-year. The job responsibilities included the following:
- List out the responsibilities
Signed off by the supervisor with contact information in the signature.

Hi, I have a question, at my last job I worked as a Research Technician for a Professor of a university. I had a supervisor. However, technically my employer is the university and not the Prof, and so he is the "supervisor". It says in my Offer Letter that prof is the supervisor, but I'm not submitting my Offer Letter with my application as it wasn't asked for.
