Documents requested - Release of Information and one other.

Hi,

I submitted Permanent Residency Application about 2 weeks back. I have received a letter from CIC asking me to submit 2 documents as follows.

1) 1106580*** - [Name]
2) Release of Information IMM5475: A signed and completed Authority to Release Personal Information to a Designated Individual form (IMM5475) bearing original signatures.

First document requested is a number followed by my name and I have no idea what it means.
Can someone please help me find out what this documents is and where I can download?

I downloaded the second one online. Could someone point me to a sample filled up document so I can fill it up?

I need to submit these within 6 days. Please assist.

Have you requested that someone be allowed to access your information? A lawyer or consultant by chance?

No I haven't made any such request. What is the purpose of this form? Is it ok if I don't submit this?

the first number is your UCI number. You are only submitting the IMM5475 . However since you didn't authorise anyone else to view your file ,you should not submit his file instead send a LOE stating you have not approved any person yo view your file
