I received my ITA yesterday via FSW and i am completing my application. Need help on answering following questions.
1. Has this person ever used any other name? (required)
In one of my degree, my name is printed along with father name. The name as per Certificate was written same in ECA report as well. As per my passport, My father name is not part of my name.
Should i answer Yes to this question? If so, what document do i need to provide?
2. Address of Correspondence.
P.O. Box (required)
My Correspondence address does not have any PO Box number. What should i write here?
3. Can Address of Correspondence and Residence be different? I want to give my office address so i can receive mails easily. Do i need to provide any documents for this?
4. In which province or territory does Rehan intend to reside? (required) & In which city/town does Rehan intend to reside? (required)
We can only select one option from this. Can i live in a different province and town than what i declare here?
5. Have you listed all of Rehan's periods of military service or government positions (if any), and all of the organizations that Rehan belongs to (if any)? (required)
I have never been associated with military or government. Do i select Yes, No or Not Applicable?
6. Please tell us about Rehan's immediate family members that have not been identified in the application.
I understand i need to give my siblings and parents details here. But i am a single applicant and they won't be accompanying me. Do i need to provide any documents related to my parents and siblings? or just the information required would be sufficient?
7. Has Rehan, and if applicable, Rehan's family members taken any trips outside their country of origin or of residence in the last ten years (or since their 18th birthday if this was less than ten years ago). Include all trips: tourism, business, training, etc. (required)
What does the term "Family Member" mean here. My marital status is Never married/single. All my international travels were before the age of 18 with my parents and siblings. Do i need to provide trips details of my parents and siblings? If so, Do i need to upload their passports too?
8. Has Rehan had any serious disease or physical or mental disorder? (required)
Does this also include past history of tuberculosis? If so, no proof of treatment is available as it's been many years. How to tackle this?
