Study Permit from within Canada and health related jobs

Hi,

My wife had a Work Permit (expired on Oct 2017) under the IEC working holiday program and, since we are NOT from a visa exempt country, she also had a Work TRV (expired on May 2017). She applied for a Study Permit 5 weeks before her WP expired. She's got her SP.

Now she's a full-time student at a participating post-secondary institution and want to work off-campus up to 20 hours per week in a job related to one of the following categories:
  • Medical esthetician.
  • Spanish Teacher: working with young kids.

She has NO medical exam performed by an IRCC authorized panel physician (doctor) within the last 12 months. Her most recent medical exam was in May 2016 when we got our IEC work permits.

She hasn't lived in a designated country or territory for more than six months in the last year. More precisely, she's been in Canada for the past 12 months.

Questions:
  1. Someone told her she needs a separate document, apart from the Study Permit, to be able to work in any type of job. Based on her SP Conditions: MAY ACCEPT EMPLOYMENT ON OR OFF CAMPUS IF MEETING ELIGIBILITY CRITERIA AS PER R186(F), (V) OR (W). MUST CEASE WORKING IF NO LONGER MEETING THESE CRITERIA. Based on this, I would say she can accept a job right away as long as she's studying. She doesn't need a separate work permit. Is this correct?
  2. Based on the eligibility questions on the CIC website, she MAY work off-campus up to 20 hours per week but it doesn't say anything else. Can she work on any type of job?
  3. Since we will travel abroad soon (not to the US) and come back to Candada so she will apply for a new TRV and we need to know if she should request for medical instructions or skip the medical part. Her field of study is related to HEALTH SCIENCES (this is specified on the SP). I'm not sure if a medical exam is required in her case regardless of the time spent outside of Canada (a week or two) at the time she's going to use the TRV to come back.
  4. What type of TRV will she get? A visitor's V-1 visa? A study Visa? Multiple/Single entry?

Here's what R186 says:

186 A foreign national may work in Canada without a work permit

(f) if they are a full-time student, on the campus of the university or college at which they are a full-time student, for the period for which they hold a study permit to study at that university or college;

(v) if they are the holder of a study permit and
(i) they are a full-time student enrolled at a designated learning institution as defined in section 211.1,
(ii) the program in which they are enrolled is a post-secondary academic, vocational or professional training program, or a vocational training program at the secondary level offered in Quebec, in each case, of a duration of six months or more that leads to a degree, diploma or certificate, and
(iii) although they are permitted to engage in fulltime work during a regularly scheduled break between academic sessions, they work no more than 20 hours per week during a regular academic session; or

(w) if they are or were the holder of a study permit who has completed their program of study and
(i) they met the requirements set out in paragraph (v), and
(ii) they applied for a work permit before the expiry of that study permit and a decision has not yet been made in respect of their application.

She's been offered a job as a teacher.

So my understanding is that she should call/email CIC and ask if she can accept the job.

Question #4 was more for me, because I have a valid visitor's multiple-entry TRV V-1. I got this before my work visa. Now my work visa expired. I'm waiting for my new Work Permit, but I don't know if I'll need to ask for a new TRV or if I can use my old TRV V-1 which is valid until 2021 in order to come back as a worker.
