I didn't declare it in my App as my wife is the one sponsoring me and the refusal was 2015
Big mistake. You have committed misrepresentation and it is very possible that your app will be refused and you will receive a 5 year ban from Canada.

Jesus christ ... I was not asked to declare it anywhere in the application and my lawyer never told me anything of such. Do i need to write AVO to let them know of this because I'm really scared and worried ... My passport got to AVO today.

Clearly on the Schedule A form it asks if you have been denied entry or visas or that sort of thing from Canada or any other country. If you put no, then it is clearly misrepresentation. I don't know if writing them to let them know about this would do anything at all, as the misrepresentation has happened already. You lied in your app, and there are consequences for that. Hopefully it'll be okay somehow, but the fact is that you did commit misrepresentation.

It was my wife that filled that form and I'm sure she filled no ... I just sent a message to my lawyer and he said the same thing that if I filled no then it's left to the reviewing officer in charge to make decision ... I don't know if writing them will do any good as I'm losing it right now ... If I knew there was that part in the form I would have delayed my passport then look for a way to call them before sending ... in cases like this what's the worst case scenario??...Has anyone ever been lucky in this situation before ... Pls I need help from this forum.

IRCC won't care if you say your wife filled it out. You should have been the one filling it out to make sure the information was accurate. You should try to contact IRCC in every way possible and have your wife contact her MP.

Worst case scenario is exactly what I said above: refusal of your app based on misrepresentation and a 5 year ban from Canada.

Just told her and I'm also thinking of re-filling the form and sending it out to IRCC with cover letter explaining that it was a mistake ... what do you guys think?

I've drafted the cover letter explaining to them that i made a mistake and i also refill the form ... Should i send it to Accra Visa office since my passport has been there since yesterday via courier or email?... Should my wife also send to IRCC just to be on a safe side ... I'm freaking out badly and i couldn't sleep all through yesterday till now

Email as soon as possible then send courier letter. What is critical is that you do not try to land.
