Which all provinces offer Medicare from day 1?

Most probably Alberta does. Also Quebec offers medicare from day 1 to pregnant women.

Wondering if their are any other provinces.

- Alberta (For eligible residents arriving from outside Canada only)
- Nova Scotia (for returning Canadians and PRs moving from outside Canada only)
- Prince Edward Island (for returning Canadians and PRs moving from outside Canada only)
- New Brunswick (for Foreign Nationals and PRs (including Citizens) moving from outside Canada only)
- North-West Territories (for Foreign Nationals and PRs (including Citizens) moving from outside Canada only)
- Newfoundland & Labrador (For eligible residents arriving from outside Canada only)

Yes as long as you are permanently relocating to Nova Scotia. You can not give birth and then move to another province. There are residency requirements for provincial healthcare and they are particularly looking at pregnant women landing in provinces who offer healthcare right away.
