Schedule A Background Declaration Form submittal

Hi all

I had applied for Canada Permanent Residency on August 9th, 2017. I had also made the Permanent residency required payment as well.
Now I receive a message in my account indicating a Schedule A Background Declaration Form submission indicating the following requirement.

Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature covering the period of 2013/11/13 to 2014/04/24 and 2016/03/20 to 2017/08/09.

Now My doubt is, where do I submit the completed declaration form? They are asking me to upload the document in myCIC account. But I do not see any option to upload it.
Kindly guide me through the process.

There must be a new placeholder when you click on view submitted application. Submit and make sure you transmit it properly after uploading and esigning it.

If you go to the documents submitted in your application you will see a place to upload it. I have received that request too.

Sorry for troubling...but I still could not find the Docuemnts submitted section in my GCKey application page. If there was an option to upload a screenshot I would have shown it. Currently it is shown as 'No Data available in Table' under Continue My Profile/ Application Section. It would be really kind of you if you could elaborately explain where the uploading option is.
