One and the Same Person

Dear all,

My Name is Rakesh Khanna and my last name is Krishnamurthy. So my name in passport and CIC and other places appear as "Rakesh Khanna Krishnamurthy". However, in my educational certificates and Employment Certificates the name appears as "Rakesh Khanna.K" and "Rakesh Khanna" respectively. While filling up the ITA form should I submit a "One and the Same Person Affidavit".

Admins - I have searched through this forum to find relevant threads but could not find one. If there is one already please point me to it and delete this one.

There is option of OTHER name where you can mention your other nam e"Rakesh Khanna,K" . you have another option after ITA , you will explain in LOE your situation and upload affidavit along LOE .
