Thank you for the reply. I have already sent this form, this is supplementary form for travels. They want Supplementary information form for Afghan Nationals, I don't know where to find this,

i can help you , i did this form when i applied it .. go to this site i give you . but you should to print all Appendix A and then you can take just for Afghanistan not pakistan , because cic make fix both to gether .

Zora, can you please tell me how to submit this form back to the Visa Off? Should i send by email or by post to New Delhi visa office?
to be honest , i sent all this forms with the package of my applications first time. and i dont know how you can send it . just i can help you other way ..

If you lived in Toronto i can give you number Afghan women's organization to call there and they help you very well. but if you cant call and is not possible to call i can call and asking about your Issue ???

Salam Zora jan. Hope you are doing well. I just tried to call that number on extension 58 but nobody picked up the phone. Is it because the office is not yet open?
